Created a new thread. Posting on old threads can be confusing for members reading the original thread.
When the engine is cold, remove the radiator cap and verify the radiator is filled with coolant. Air in the cooling system can cause idle problems.

What do you mean you have to hold it in the start position to get it to start?
A flaky ignition switch can possibly behave like this: (sorta common for Hondas)
- Key to START position - it fires like it is going to start.
- Release key to RUN position - shuts down unless you hold it just a tiny bit.
What's happening is the flaky switch is actually shutting down the engine. You can keep it running if you don't completely release the key, just hold it a tiny fraction away from where it would go if you let go of the key.
For the flaky idle speed... what PA said plus you should use the bleed valve at the thermostat housing to get the air out of that housing.
