Depending on what spark plugs are in the car, the service life could be different. I'm using the manual I have for this gen Accord, posted the part number of said plugs as the base of my comment/suggestion.

Worn rotor and cap

I think this is the culprit. Put new cap and rotor on last night and drove for 15 minutes it was smooth with no misses and idled perfect and didn't throw a code. When I zoomed in it is really pitted on rotor and on the cap. So with the pitting i assume it was throwing out random sparks across the posts causing the problems. I'm going to drive to work Friday which is 26 miles away to give it the real world test, crossing fingers. I will also grab some fresh plugs this weekend that you gave me the numbers for. Figured if these pics helps someone out then great.
