Honda Pilot

It's 2003 Honda Pilot 258,000 miles on it. What does this code mean?

Are you leaking transmission fluid? Have you checked the level? Check the harness. The part that it's saying is bad: 28400-P6H-013. With that many miles I'm surprised you don't have a P0740

Shift solenoid could be stuck due to debris or the filter clogged preventing fluid flow. I'll take a look at how the mechanics of the solenoid are monitored, because I'm guessing there is a pressure switch downstream that verifies the solenoid operation.
