Default possible trans issue

I have a 95 accord and its the lx model 2.2 sohc non vtec. Whenever i switch gears, and apply the accelerator my rpm will go up about 500 or so and i wont accelerate. Its as though i pressed the clutch again after shifting.

ex: 1st gear 2500rpm 15 to 20mph
shift
2nd gear 2000rpm (and when i apply the accelerator the it will shot up to about 3000 at the worst ive seen it go up to 3800 and ill only be going like 29 mph)

Im thinking bad clutch... also i used to grind the gears every now and then because the trans fluid was low. Which i found out when i did a drain and fill. No leaks on trans. Does this sound about right? time for a new clutch?

Yes, slippage under load is clear indication of failiing clutch plate.

good luck
