So my transmission kind of jerks when shifting under a light acceleration (2800 rpm shifts) but if I accelerate hard and make it shift at like 3800 rpms it shifts ALOT smoother and transitions from one gear to the next better. Any ideas as to why this would be?

 

Check the mounts. Check the throttle control cable....assume the fluid has been replaced on some schedule.
