In 1998 & up, the heater temperature control is driven with a little motor. So if you twist the **** all the way to hot, the actual valve in the water hose doesn't move until you switch the key to ON (don't need to actually start the engine).
You need that valve open so antifreeze gets into the heater core when you fill. Otherwise you'll probably have large air pockets in the cooling system.

Another follow up, it seems as though the car is not doing the crazy idle anymore. It did it yesterday morning, and then it didn't do it for the evening commute (2 miles) and didn't do it for the morning commute today. We replaced the O2 A/F ratio sensor last night and cleared the CEL for P1167, so we'll see if that comes back.

Well, it was all in vain. Her car was totaled a couple hours ago in North Carolina in a big pileup (she is doing fine)! All of that for nothin'! I hope it can benefit someone here, though!
