It might be a bad water pump (bearings bad), instead of the drive belt tensioner. Your K20 engine is similar to the K24 engine.

This video link is for water pump replacement. It also has the links for the separate drive belt removal, power steering pump removal, drive belt auto-tensioner removal (which are all required to replace the water pump).


If it is only the drive belt auto-tensioner, then see this video:

 

It sounds to me like tensioner may be hitting the stop due to stretched belt. If belt is old, you might try a new belt.

I can see the tensioner bouncing in the video.
