I'd recommend cleaning as much as you can. The intake can get like that if you have worn engine rings and a lot of blow-by of oil mist and some leakage from the combustion chamber. The PCV valve could also be stuck open and cause the intake to suck in the oil mist from the crankcase. The PCV valve is cheap and easy to replace, so you may want to start with this.
I'd also clean the TB, since you intake looks very dirty.
Just over 200k it runs fine no miss or smoke and now I think I opened another can of worms and still haven't solved my original problem 😂 I'm going to clean manifold upper and lower and replace all the gaskets on the way back up to the air intake I'll pick up a pcv valve as well. What are your thoughts on the throttle body on eBay ? For right under 100$ they have all new sensors on them would any of this cause my idle surge?
I'd hold off on guessing and throwing parts of unknown conditions at the car. There is a port on the lower left side of the lower intake in your picture that you should clean out. That is the EGR port. The port also goes through the upper intake. You might as well clean that out while you are in there.

Agreed. Especially if you have the top of the plenum off. It might even be a good time to replace the IAC valve, since accessing those little M6 screws is kind of tough (not a lot of room) BTDT before on my wife's 00 Accord. Getting those small hoses off the IAC isn't easy either. When you go to remove the plenum top, unbolt the throttle and cruise control bracket, then you don't have to mess with readjusting the cables.

I picked up some degreaser from work industrial grade I work industrial maintenance on going to remove the lower plenum and clean them both up really good and install new gaskets the p.o stated he changed the iac and it worked for a few days and came back and he said the owner before that did the same but I have one coming just for sport in case that is the problem and they both received duds it's not just an idle surge it surges anytime the idle falls under about 3000 rpm ? Would the iac cause that as well or tps possible?

On my wife's car, that the IAC went bad on, it was doing it at 1200 to 1500 rpms when it should idle at 750. I changed the valve, then let it do a "re-learn", and we haven't had an issue with it in the last 3+ years now.
Sorry for the delayed response I have been really busy with work and it's been raining horribly the past few days. Anyway I managed to clean the intake up good the egr ports we're clogged almost shut I also noticed that the head looks new from the backside I checked the timing and it's good I think the manifold gasket may have been leaking a tad bit. But I also checked the tps with a meter and I get 0.14 at idle 0.67 at wot which may not be correct according to what I've read I have ordered a iac valve and it will be in in the next day or two in going to work on putting it back together it will probably be a few days but I will report back
WOW, yours was very dirty/black looking inside. When I did my wife's car (at 224K miles) the inlet ports to the engine looked cleaner than yours did. Hers also had a plugged EGR port, and after I put it back together, the valve went bad, so it got replaced too.
Well I didn't forget to post what happened I just now got it back together so. I removed and cleaned the upper and lower intake out, installed with new gaskets and replaced the knock sensor because I broke it on manifold install. Cleaned the egr valve I also Cleaned the throttle body adjusted the tps. Replaced the iac valve and the thermostat. I didn't like the way the one in there looked I also put a new valve cover gasket on it. After bleeding it out and firing it up it tried to surge a little bit. But I forgot I had left the idle air screw out a good bit once I turned it a half turn she runs like a champ. Just idling in the driveway with occasion revs I'll take it for a good drive after I change the oil and put a new o ring on the distributor. So my problem is solved I'm sorry I have no idea what it was one of the ten things I've done I'm sure. I'm still hitting a bald spot after I rev it when it idles down it seems to hang up at 1200 rpm for a second then idles fine maybe the ECU needs a few drive cycles? Or possibly caused by the bad o2 sensor I have ?
