codes detected for accord 98 jerking at speed 80km/hrs and 100km/hrs

Good day to you all in the forum, I have been on this issue for some weeks now. But today i was able to carry out a scan on the car. The scan result gives the following codes: P1381, P1491, P0301, P0302, P0303, P1456. It seems the code P1456(fuel abnormality) is what is causing the jerking. Please i want to know about this code P1456 and how can we resolve it. To me it seems there is a leak within the fuel system that is causing the jerking between 80km/hrs and 100km/hrs. P1456 - check your gas cap and make sure it is tightened, or that there is no cracks in the rubber ring...

P1381 is intermittent CYP (Cylinder Position Sensor) fault. It can be faulty sensor, wiring, connector, or PCM. This is likely source of missing and will need to be diagnosed to isolate fault. A multimeter is required and knowledge of finding shorts/opens and measuring resistance of sensor.

P1491 is EGR insufficient lift. This should be corrected but is not likely source of poor performance. Possible EGR vacuum supply fault, EGR vacuum solenoid, or EGR valve.

P0300-0301, etc are likely result of P1381 fault and should correct if CYP sensor fault is addressed.

P1456 is evap system fault, leak of vacuum from EVAP system. It can be fuel gas cap or other problems. Not likely source of poor performance.

you maybe correct the P1381 might be the problem, but i hope i will not change any sensors, thanks

Greetings to you all in the forum, we decided to do some work on my honda accord 98 today, we checked the distributors housing, connectors, plug wires all seems to be ok by the technician, we also look at the injector nozzles we discover some part were covered with carbon soot. we have to clean it with a solution injector nozzle cleaner to wash out all carbon deposits present inside. I changed all the 4 spark plugs, did some adjustment in the throttle cable. when we test drive the accord it droves smoothly with no hesitation even at above 100km/hr and i did not experience any jerks or whatsoever. Is it possible that the scan results might be wrong? because we got P1381, P0301, P0302, P0303, P1456. Code P1381 represents cylinder position sensors. only what i did here is to change one of the plug wires connecting to the distributor because it wasn't covering one of the plugs very well. please how do you people see it. THANKS
