Okay, I have driven it some more and have the following codes. P0300, P0301, P0302, P0303, P0304, P0305 and P0306.
Any thoughts on what may be causing this which we haven't tried? Are there any sensors which would impact all of the cylinders which wouldn't trigger a code of their own?

I don't like suggesting throwing parts at a problem without testing but here I go off into left field.
Buy, I know, one new ign coil (rockauto is less than $45, plus shipping, minus any discount code you can find - they e-mailed me one just this week).....or go to a bone yard and grab a used one or two from there? Place the new coil into each cyl one at a time...new coil into #1, see what happens.
Same thing, put old coil back into #1, put new coil on #2.....same thing, old coil into #2...new coil to #3. I think you get the idea.
Again, I do not like the idea of spending ~$50 on a guess.....but you asked for for thoughts/ideas......sometimes the ecu/pcm gets "confused" on exactly which cyl is having the issue and reports all as having an issue.

Sometimes you need to admit defeat and look for help. Do you know a good diagnostic shop that can perform an indepth investigation? They have access to all the data the OBD2 system produces and should be able to identify; fuel or spark and proceed to a diagnosis.
That's what I had to do w/ 2001 Celica. It cost $95 to find out a couple of injectors were blocked. I did not have the tools to investigate this problem and had discounted injectors as a possible solution.
