Default Brake noise on hard stop

My 2010 accord v6 sedan has weird grinding/clunking noise on the front wheels when hard braking especially on slope and rough road. There is no noise on flat road. The pads are about 5mm thick but i do feel slight vibration on the steering wheel on the freeway when braking. Any thoughts or contribution is appreciated.

Have the suspension checked (sway bar links, bushing, ect).

Also, can't remember, are the "wear tabs" on the pads touching the rotor? A vibration at that speed and under braking could be worn rotors.

the noise is intermittent. It sounds like something is in between the rotor and pad. I usually stop grandually that is why i seldom hear the noise. The wear tabs are not touching the rotors and i dont hear any squeal.
