New way for an IACV to drive you nuts.
I took my Accord to the dealer for a surging issue I could not resolve. Their proposition was to replace the throttle body. I don't think so. After taking out the TB and finding nothing I checked the IACV which I had earlier cleaned. It was sticky in its rotation of the cylindrical shutter valve. So I took the coil off to discover that the upper portion of the shaft's permanent magnet had partially shattered and had fragments on it that interfered with its smooth operation and its complete closure -- a necessity for getting the base idle set.
Conclusion:
If you have the IACV in your hand take the coil off and inspect it and its driven magnet for damage.

Photo of fractured magnet in Denso IACV
You asked for it. Using my binocular microscope and a Nikon D-3200. The lighting is a bit difficult so image is not optimal but adequate. The Denso label has MX136800-1141, A A L 64 and 14 in big type and Made in Mexico.
