Strange engine stumble / hesitation

Default Strange engine stumble / hesitation

My Accord has a strange hesitation that only happens on a cold start, and it only happens sometimes.

I'll start the car and drive off. Everything is normal at that point. After about a quarter mile of driving, the engine stumbles while accelerating for about a second, then it's back to normal for the rest of the drive. Doesn't happen again during the drive, and doesn't happen again under warm start conditions. Only happens sometimes on cold starts. And it only happens about a quarter mile after I take off.

Any ideas where to start diagnosing? I'm perplexed.

Do you have a check engine light on? with cold starts im thinking maybe EGR valve issue?

No, I don't have a check engine light on. Just to be safe I checked for any stored codes and there weren't any.

If I had a failing EGR valve or clogged EGR ports wouldn't the symptoms always show up at a certain RPM range, instead of just one time after each cold start?
