It depends upon which model and engine you're asking about. Case in point, my 2001 Accord Sedan came with 15" wheels for pretty much all trim levels, however, the 4-Cylinder models had 4-Bolt wheels while the V6 models had 5-Bolt wheels.

#1. I don't know exactly. Are you asking when the brake calipers became large enough to prevent installing 14" wheels? Or when Honda began using 15" as stock wheels on certain trim levels? 1997 Accord DX came with 14" wheels but the 1997 Accord EX came with 15".
I had a 1998 Accord EX 4-cyl and the brakes were too big to fit 14" wheels.
#2. Everything had 4=bolt up through 1997.
Beginning in 1998 the V-6 cars had 5-bolt while 4-cyl had 4-bolt.
Beginning in 2003 ALL Accords had 5-bolt wheels.

1998 thru 2002 Oem Rim 14" and 15" - 4 lugs 4cyl
1998 thru 2002 Oem Rim 15" and 16" - 5 Lugs 6cyl
2003 thru 2007 Oem Rim 15" , 16" and 17" 5 Lugs
