You would get the part from Honda. Due to shipping costs, internet Honda dealers may not beat the local dealership costs by much, if any.
The part number is generally not in the service manual. Search for the part number at internet Honda dealers such as Bernardi Honda Parts, Honda Part Now, etc.
In this case, google is also your friend. Just google 2004 Accord air mix control motor to see how to replace, it is just unplugging an electrical connector and removing 3 Phillips screws.

I would check w/ a local Honda parts suppliers. I looked on online sources and they list two part no's that are very similar (79160-SDA-A01, 79160-SDC-A01). No idea of difference.
Lowest cost is repairing the part or used part (ebay $20-$30), otherwise $60-$80 for new part.
