Cold start trouble

I recently moved from Las Vegas to northern Utah, so I've gone from warm to very cold and I think the temperature has brought out a new problem in my 92 Accord. On a cold start it seems like it is taking awhile for all 4 cylinders to start firing. Its a consistent roughness that lasts from 5-20 seconds. After a little convincing it runs perfectly and there are no problems after that. The car has almost new ngk plugs and wires and there isn't a problem when its warmed up. I appreciate any ideas any of you may have!

are you saying that once it starts running it's rough for 5-20 seconds or it takes 5-20 seconds of turning over to start?

either way if it's alleviated once it's warm then a block heater should help

Nate, is it possible that his HITV is messed up and not holding the idle high enough for the initial start? The 5-20 seconds thing is why I'm thinking of the high idle. Also, a rough idle usually disappears if you give it a little gas.

The car starts right up, its just rough. The idle speed seems ok, but again, its rough.
