Default Fast Idle Valve Help

Okay, the service manual finally came today and it says that my FITV could be the cause of massive surging on cold starts. Question...I cleaned it twice (not very dirty in the first place)...does it need to be screwed in further or should I replace it? Could the wax be old and bad?

The service manual says to check air flow at lower throttle body port. There is no vacuum here at startup on my car...but a lot of vacuum at the upper port. Any ideas???

Saw that and did it...it's in as far as it can go. But I can't figure out why it appears to be closed ALL the time (the bullet tip is always flush with the end of the plastic).

Can someone please confirm that on cold starts air should be sucking through the lower throttle body port?
