Default hub assembly on a 91 accord

hello about 3 months ago my bearings went out so I replaced them. when I replaced them I noticed that they did not have to be pulled out of the wheel hub itself it just popped off with the pry of the screwdriver. I thought nothing of it because they were worn. when I put the new ones on I packed the hell out of them with grease and popped them on the hubs. When the new bearings were on I notced that there was alot of play between the hub and the bearing, I tried pounding it in as hard as I could but there still was play so I put it on like that. The car drove great untill yesterday it started making a grinding noise like it had done before 3 months ago. what did I do wrong??
sorry if this doesnt make sense in a little frusturated at the whole situation. what can I do differently so that this doesnt happen every 3 months??
thanks
