Default Transmission problem? 92 honda accord dx

So, my 92 Honda accord 2d dx has been slowly starting to get more of a emphasized switching of gears (it is an automatic) sometimes it jerks a little when going up the first couple of gears or revs a little too high between shifts. I usually take my foot off the gas for a moment when I know it's going to shift up and it makes it a little smoother of a transition. Recently it also has been having this thing when it gets to cruising speed at about 55-60 mph the acceleration feels as though it has a very slight stutter (almost feels like when your on the freeway on a really windy day, so I'm not sure...it's hard to tell) It doesn't make any abnormal sounds when it does either of these things...any suggestions?

Has the fluid been changed lately, check the mounts, check the throttle control cable.

Any recent work done to the car.

Also Check for trans codes (see diy forum for procedure). Some codes don't cause D4 or S light to flash.
