Default worn out ckutch

The clutch in my 97 accord lx feels different lately. I'll try to explin this the best way i know how. As soon as i release the clutch the car falls into gear. It fels worn out. The distance it takes from when i start to life my foot from off the clutch to when it actually falls in gear has reduced dramatically. Any idea what this may be from and also any solutions to fix this. If its at all possible to DIY then please help out out with descriptions for that as well. Thanks ppl.

aren't the clutches self adjusting?

how many miles on the car (or KMs)?

yes they are, try bleeding the clutch system and see if that helps.. might have air trapped in system
