Default 95 accord transmisison problem

I have a automatic 95 accord. My car randomly shifts gears while i am driving. Sometimes when i slightly apply pressure to the throttle it will rev the engine and then when the rmp gets real high it will shift, but i barely touched the pedal or i didn't do anything at all and it will spike like that.. I might be going up hill and it will shift down and the engine will rev. Also if i slightly let off of the throttle and then apply a little pressure back on the car will jerk and the rpms will shoot up then it will calm down. Does anyone know what is going on with my car? Is this enough info? please help.

One thing to check is the cable from the throttle to the auto-tranny. Cant help much, cause I never had an automatic from those years. The cableadjustment is important so the tranny knows the throttle position.

Refer to this post for cable adjustments; also check for codes in the TCM.
jumper the BLUE 2 pin connector behind the lower right corner of the glove box, turn on the key and watch for 'D4' flashes
