Default 97 SE grinding on 1st acceleration

My 1997 Accord SE with 199,000 miles just started making a grinding noise for a few seconds during the first acceleration. After that, it runs fine. Anyone have any idea what this could be? Thanks.

4-cyl? V-6? Auto or manual tranny?
If auto, do you give it a moment to actually engage the gear before giving it gas?

How's the tranny fluid? Got real Honda ATF in there? People have reported rough shifts & other problems with Dexron or other fluids.

Drain/fill the tranny lately? Honda auto trannys don't seem to like "power-flushing" for some reason.

Other than that, I'm no auto-tranny guru, so I hope someone else chimes in.
